Well, Cora, there are a few options, though not many, available to you. There are chemicals in just about everything, from birth control pills to spermicides to the latex used in condoms and diaphragms! I am excited about a new method of Natural Family Planning called PFT 1-2-3 ((Personal Fertility and Reproductive Health System). I spoke personally to people involved with this product, and it looks quite promising. By simply licking a hand-held plastic triangle outfitted with colored lenses, you can determine your day of ovulation and fertile days. Thus, it's helpful not just for those avoiding pregnancy, but also for those planning a pregnancy. Reportedly, some gentleman used the device on a date to determine if his girlfriend was "safe" for the evening. Of course, it does not detect diseases, just the chance of pregnancy.
Wow Laurie, that sounds really interesting. Where could I acquire some of these triangles and about how much do they cost? Also what is the rate of failure so far? Cora
Good to hear from you again, Cora. Here's the scoop on the triangles: They cost about $75, and they are about 97-98% effective in predicting the time of ovulation, according to the literature. Dear Victoria, As for HIV, while there continues to be new breakthroughs almost monthly concerning drug "cocktails" (mixtures) which boost compromised immune systems, those at risk continue to be the same as when this whole thing started - those who practice risky behavior. Male homosexuals and I.V. drug users who have not or will not protect themselves during certain practices, and those who sleep with them, continue to contract the disease. At least we don't have to worry about mosquitoes anymore, but the jury is still out on toilet seats (just kidding - but use those paper covers anyway).

I heard that in the olden days women would use alum because it was a vaginal constrictor and it, at least temporarily, caused the vagina to contract. Hi again, Cora. Your question about vaginal tone is a good one. Unfortunately many women do complain about feeling "loose" down there, particularly after having had a vaginal delivery. The vagina does naturally have a large potential space so I can't imagine a penis alone would make it any larger, even a penis belonging to a man who is well endowed. A baby, however, can stretch the vagina and the opening to the vagina, called the introitus. I've seen women with gaping vaginas where you can nearly see the cervix without a speculum. Most of these women have had multiple pregnancies (like five or more children). What can be done? I'm not too familiar with surgical procedures. I imagine you could tighten the opening by pulling the excess skin together. I would first suggest exercises to strengthen the pelvic floor muscles. One kind of exercise is called Kegel exercises. A woman can do these by sitting in a chair with her legs slightly apart. Then, she should contract the pelvic muscles by pressing the buttocks together as though stopping and starting the flow of urination. She can check how strong the muscles are by tightening them around her finger. The more she is able to squeeze her finger with the vaginal walls, the stronger the muscles. Repeat these throughout the day, doing them twenty times each set, four times a day. Many women report success with this and they don't have to resort to surgery. It's best to avoid the knife.

Dear me, where to start? Your question opens up a whole "can of worms" so to speak (although another nurse actually saw this can), but I'll try to narrow it down . . . could it have been the guy who hadn't washed his penis for two weeks, got mold from his toilet on it, then wanted to know if it all was sexually transmitted -- even though he hadn't had a partner in some time, needless to say? Or was it the feminine product that some lady forgot she had inserted a month previously, and the ensuing stench when said green slimy item was brought out into the open air? Perhaps one of the many examinations where I suddenly notice tiny somethings moving in the genitalia area . . . well, I could just go on and on. It seems like I have many gross stories, but for some reason l can't think of any more right now. Hope this holds you till next time!

I'm back. I apologize for the delay. No excuses, I'm just plain lazy. Now, I have many questions to respond to.
Let's see, first I will talk about the new sanitary protection item on the market . The "Instead" was a real breakthrough development in the last year. I am told it was designed by a woman athlete who needed something to use on her menses so she could "jump around a lot". It's basically a size 80 diaphragm with a little bit larger pouch to cup the blood as falls down from the uterus, through the cervix ,and into the vagina. Although I personally found it to be usable, I have heard several complaints from a few women who have tried it. One complaint is that when it is removed it can be a bit messy, blood splattering everywhere as the spring releases. If you are careful, you can prevent this by cupping your hands around it as you remove it. Another woman said that it leaked while she was using it. Apparently her cervix is tipped and the Instead wasn't covering it adequately. It was also uncomfortable for her. Luckily, she is a Nurse Practitioner and she was able to remove it herself without any assistance. During my practice, I've had to remove stuck diaphragms from a few patients so I imagine I may see a patient or two in the future with a stuck Instead inside her (not any worse than other items I've retrieve from other vaginas). Another concern I have, which may be unfounded, is the possible risk of endometriosis. If the Instead is used at night or during intercourse there is the possibility of retrograde menses. This is one theory of the etiology of endometriosis. Endometriosis is the condition whereby uterine tissue implants itself outside the uterus and can cause a variety of symptoms from painful intercourse to painful menses to a plainly painful abdomen. Other than these concerns, I think the Instead is great. Perhaps there is a tiny reduction in the risk of toxic shock syndrome compared to using tampons.
Next was the question about the use of midwives during pregnancy. I think midwives are the best, and if I were to get pregnant I wouldn't hesitate to have one for my delivery. In fact, home births with midwives have a much lower complication rate than those with MDs in the hospital. Midwives, generally are much more in tune with the whole birthing experience. They understand women's needs and concerns and are, in my opinion, better equipped to assist the woman with her birth (key word here is assist). So many doctors think they are the ones who are delivering the baby. I know, I worked in labor and delivery in the hospital. I highly don't recommend it.
Last, I finally have a question about pap smears. Of course the woman who wrote this question was the one who first taught me about pap smears and she knows the answer already. What is the difference between a Low Grade Squamous Intraepithelial lesion verus a High Grade Intraepithelial Lesion? The latter is much worse to have on a pathology report. It all has to do with the amount of tissue at the cervix that has dysplastic (pre-cancerous) tissue. Low grade means that approximately one third of the epithelium (cervical tissue) is involved. High grade means that more is involved. The worst high grade report can be a precusor to invasion, meaning carcinoma or cancer (not good). It's important that a woman who has a pap smear with an abnormal reading, such as the above, have further examination. A colposcopy is recommended. I do a number of these procedures each week. During colposcopy, a clinician looks at the cervix with a high powered magnification instrument, called a colposcope. If she sees any abnormal areas, she biopsies those areas. Biopsies then confirm whether or not the tissue is cancerous.

Now, even though I love my partner a great deal, sex is one of the last things that comes to mind. I had a check-up recently and medically everything seems fine. What other kinds of things could cause this loss of appetite?

Dear Sabina There are so many causes for a decrease in libido. Since everything is okay medically, perhaps the cause has to do with emotional or psychological issues. Are you able to spend much time together where you both don't feel tired or stressed out? Exercise and outdoor activities done as a couple can be a big booster of the endorphins which can promote sexual feelings. I've heard that Ginko Biloba is a herbal sex stimulant. But really just spending time together doing fun activities is the best way I've found to increase sexual desire. Let me get back to you with other ideas. I'll ask around. I'd like to know more ways to help this problem as well. Any suggestions out there?
Try cutting down on those hypertension medications, I say.

At one point they were saying the HIV virus takes only about six months to show up but recently I have heard report that it could take years for it to register in your blood, which is what they had been saying when they first discovered the disease. What have you heard about this?

Yes, it is true, you can get gonorrhea in the eye. It tends to grow easily on any mucosal surface which includes the eye. However, this form of gonorrhea is very rare. The more common areas to be affected are the vagina/cervix, the urethra, the throat and the anus.

Dear Mistress V. I haven't heard of it taking that long to test for HIV. In fact, one of the newer tests, the PCR test, can detect the antigen (the virus) within days of exposure. This is the test they use for rape victims and people who need testing and results done immediately. I think it is expensive though and not readily available. The only answer I can think of that might be related to what you are asking is that maybe there are newer strains of the virus that are harder to test for, thus staying "dormant" for some time before they can be detected by a blood test.

Dear Mysterious Stranger -- Yes, believe it or not, men can and do get yeast infections. No special seminars or training to get this. We commonly refer to it as "jock itch". Yeast, yeast, yeast. It's everywhere! It's in the foods we eat, the beer we drink. It's not sexually transmitted but skin-to-skin contact with someone who has it can be irritating. It lives in the throat for many people and sometimes oral sex can promote its growth in one's partner. What can prevent and treat yeast infections? Besides the usual assortment of antifungal cremes and pills, some alternative treatments include acidophilus (found in yogurt), garlic supplements and tea tree oil. --pap

Of late I found myself itching around my vaginal/inner thigh area. I ignored the itching for some time but eventually I noticed that my inner leg creases were lined with dry, scaly, flaking skin. It left pink patches of the skin behind when it flaked off (they didn't feel raw though). This problem has made me very nervous. What is it? What should I do now? Dear Liz - - The most common cause of vulvar/vaginal itching I encounter with my patients is candidiasis or just plain "yeast". You can try to help alleviate the situation by putting an antifungal creme on it, like "gynelotrimin". Mycologue, a prescription creme might also help. If the symptoms persist, it could be a variety of conditions from a simple allergic reaction to something like a new soap or detergent. Sanitary pads often have perfumes and other chemicals that can irritate the skin. Psoriasis is another condition that has symptoms like you are describing. You may need to consult a dermatologist. Hi All! Sorry it's taken me so long to respond. I had a few things occupying my mind the last few months, like my wedding and honeymoon. Now, I see that there are a few questions I've left unanswered for quite a while, one about HPV and one about doing a pelvic exam on a virgin. First, I'll tell you a little about HPV (human papilloma virus). This is a wart virus and there are many different strains of this particular virus. Some strains are pretty benign but can be problematic because they cause unsightly warty growths on the genitalia. Some strains are invisible to the naked eye and some strains are more virulent and can promote cancer - cancer of the vulva , of the cervix, of the vagina and of the penis. It's difficult to know which strains you have. Getting pap smears and speculum exams regularly helps in diagnosing abnormalities. HPV is a tricky virus. From the time of exposure to the time of developing warts can be from weeks to years. Experts surmise that up to eighty percent of the population have been exposed to this virus, not all people developing symptoms. Honesty, in my opinion, is always the best way to approach this dilemma in a new relationship. Unfortunately, there is a lot of unknown information in this area. Secondly, the question about being a virgin and having a pelvic exam done. I've had many patients who were virgins and I've done a pelvic exam on them. It's always best to be very gentle and to use the smallest size speculum. With care and caution, the hymen will remain intact.

Dear Laurie, I don't know if you cover male anatomy. My son is two years old and appeared to have a yeast infection, topical penis head reddish and the skin fold appeared red and puffie and slightly seperated instead of laying smoothly against each other. He was treated with topical nystatin, two weeks later he was cultered and put on bactroban topical for possible staff, which did grow slowly, then a topical anti-inflamatory. Two weeks later he's pulling at his diaper, the skin is still pinkish and separated not smoothly together as it had always been. I was referred to a dermatologish today who told me everything is normal, not to use anything on him. Everything is not okay, he's clearly still bothered by it and his skin has not returned to how it once was. I should mention my son is autistic and being treated for excessive yeast build up in his body.
